Effective January 1, 2009, MD Degree graduates requesting a verification regarding their time in the MD degree curriculum for agencies including but not limited to graduate training programs, hospital credentialing and state medical licensing boards will be charged $50 per verification form or letter.
Graduates are provided free courtesy verifications for one calendar year after the date of their degree.
A graduate signed release of information form must be submitted to and received by the SOM Office of Student Services Morgantown Division prior to the release of verification.
